Improving Access to Care: Our Progress So Far

As part of the NHS Access Recovery Plan and the goals of the North West London (NWL) Integrated Care System, Lilyville @ Parsons Green and Primary Care Network (PCN) are working to improve how patients access care. Across NWL, GP practices and PCNs are developing new access models to enhance availability, make better use of resources, increase the use of digital tools, and provide more proactive care for those who need it. We are committed to helping you get the right care, in the right place, at the right time, whether that is with a GP, another healthcare professional, or through local PCN services.

Here is a summary of the work we are doing:

Care Navigation: Helping You See the Right Person First Time

We are rolling out a Care Navigation Programme to ensure all staff, especially our reception and admin teams, are trained to guide patients to the most appropriate service. This helps reduce wait times and ensures everyone gets the right care as quickly as possible.

Prostate Cancer Pilot Expansion

Following a successful pilot project, we are expanding our work on early detection of prostate cancer. This aims to improve health outcomes by diagnosing and treating cases earlier.

Promoting Local Services and Pharmacy First

We are developing new media content, including short videos, leaflets, and presentations, to help raise awareness of services available across the PCN. This includes Pharmacy First, which allows patients to access treatment for minor conditions directly from a pharmacist without needing to see a GP.

Digital Support for Patients

We are creating workshops and drop-in sessions to support patients in using digital tools like the NHS App and PATCHS. These platforms can make it easier to:

–          Request repeat prescriptions

–          View your health record

–          Contact the practice

Clinical Leadership and Joined-Up Care

Our PCN Clinical Leads continue to drive service improvements in key areas:

–          Mental Health

–          Women’s Health

–          Children & Young People

–          Learning Disabilities

–          Prescribing

–          Diabetes

–          Frailty

They work closely with local hospitals and community providers to ensure patients benefit from coordinated, high-quality care.

Health and Community Events

We are organising community health events focused on specific health topics or patient groups. These events help address health inequalities and support patients with tailored information, checks, and support in a welcoming environment.
